Start/Finish: St AnnChurch parking lot, 323 N. Van Buren St.Stoughton, WI, 53589

Advanced Registration: Mail entries to St. Ann Church, 323 N. Van Buren St., Stoughton, WI53589, Attn: Fall Festival 5K run/walk. Please make checks payable to St Ann’s  Festival 5K 2016.  No Refunds. Payment can also be dropped off at church office in registration box.  You may download form  at http://stannparish.weconnect.com/ or sign up at active.com

Entry Fees: $13.00 on or before September 14, 2016, $15.00 after Sept 15, 2016.  Family fee $30.00 which includes 2 t-shirts. (Additional shirts $13.00 ea). Fam Reg not available online. Contact Joanne at 608-469-9109 with questions.

Fees includes: Bib #, t-shirt and post race refreshments. Medals to top 3 youth (girls and boys, age 12 & under) & top 3 male and female participants.

Confirmations:   Will be e-mailed to all participants who pre-register.  Packet pick up is Wed. Sept 14, 5-7pm at the church gathering area. You may also register the morning of the event (shirt not guaranteed, but will be ordered).

In consideration for my rights to participate in this race event, I hereby release St. Ann Church, Festival committee, and all sponsors, and all race officials for any liability of personal injury incurred by me in participation in this race.  I further certify that I am in proper condition to participate in this race, and am aware of all inherent risks of said participation.
