This event goes to support a non-profit animal organization. We save both young and old animals and provide surgery for those that need it. We spay and neuter all of our animals.

HOW TO ENTER - Pre-register on-line at:
We strongly encourage on-line registration, but will accept by mail. We will also accept Late registrations on event day. Please bring completed form.
WAIVER: (MUST BE SIGNED) I, the undersigned, heirs, executors and administrators waive and release any and all rights and claims for damages I may incur against the beneficiaries, organizers, sponsors, advertisers, employees, volunteers and committee members, state and local governments or private parties whose property this event will be held on including, but not limited to the City of Lodi, County of San Joaquin and the Animal Friends Connection Humane Society of any and all injuries or damages suffered by me or my dog while traveling to or from or participating in this event.

- 1. PRINT all information and sign at the bottom.
- 2. If you are under 18, have your parents or legal guardian sign for you.
- 3. A pre-registration fee of $40.00 must accompany this form if returned by June 1st, 2016
- 4. T-Shirts included for all paid entries.

- 6. You do not have to bring a dog to enter
- 7. Runners may not precede their dog during walk or run.
- 8. The One Mile Jog-A-Dog is not competitive – Just for fun!
- 9. We welcome you to come with your well-mannered, vaccinated dog. Unruly dogs will be asked to leave.
